- An email address and phone number we can use to contact you if required
This information will be available to the following New Zealand Government agencies.
- New Zealand Police
- WorkSafe New Zealand
- Ministry of Health
- Ministry of Business, Innovation, and Employment
These agencies are responsible for compliance as part of New Zealand's COVID‐19 response and may be required to act on the information you provide.

Is this an emergency?
If the situation is an emergency, you do not need to complete this form.
- Call 111 if you need an emergency response from Police, Fire or Ambulance.
- Call 111 if you need to report anything that involves immediate harm to anyone (including anyone assaulting, threatening or spitting at another person)
